经过重重磨难，健四郎和尤莉亚终于迎来相聚的一刻，但是他们的时间所剩无几，在尤莉亚的请求下，健四郎开始讲述这些年来的故事：作为最强杀人拳——北斗之拳的唯一继承人，健四郎和爱人尤莉亚正准备展开新的生活。然而适值乱世，愚蠢的统治者亲手毁灭了原本的文明，活下来的人为了生存而选择暴力，杀戮成为当下的唯一准则。健四郎和尤莉亚遭遇南斗水鸟拳的阿殉，为了得到心爱的女人，阿殉不择手段，更在健四郎的胸口留下七个伤疤。痛失爱人，健四郎迎来生命的第一重磨难……
