乔治城原是一平静小镇，但忽然发生数起离奇凶杀案，而且尸首上都出现宗教仪式的钉痕；更残酷的是：凶手使用电击药品，使被害人造成麻□叫不出来；但被害过程的极限痛楚，仍有知觉！被害人自天真无邪的孩童到与世无争的神父均无一幸免，使得小镇人心惶惶，深恐惨遭毒手！乔治城的警长比尔，不断回想起十几年前被恶魔附身之神父戴明跳楼自杀的悲剧，但是不解：恶魔应该随著神父的牺牲而消灭，怎么可能又复出作祟？
电影相关：
·布拉德·道里夫扮演了一个在狱中的角色，当他被问及如何在不被察觉下进出监狱时，他答道：“It's child's play”。这句话在暗指他曾为1988年的《Child's Play》(灵异入侵)、1990年的《Child's Play 2》(鬼娃回魂2)和1991年《Child's Play 3》(灵异入侵少年军团)中的Chucky配音。
·影片灵感来自于闻名全美国的连环杀手The Zodiac。
·片中大学校长说他最喜欢的电影是1958年的《变蝇人》，而扮演校长的Lee Richardson出演了1989年的《变蝇人2》。
·影片编导威廉·彼得·布拉蒂(William Peter Blatty)曾希望片名能与他的小说《Legion》同名，在他的反对下，片名最终仍被定为“The Exorcist III”。
·最初本片中并没有Jason Miller加盟，实际上，卡拉斯神父的死尸是由布拉德·道里夫扮演，当时的场景是乔治C·斯科特正在手术台上检查神父的尸体。在试映之后，制片人决定影片中应该出现1973年《驱魔人》中的演员，于是威廉·彼得·布拉蒂重写了剧本，Jason Miller扮演了卡拉斯神父。
·影片改编自威廉·彼得·布拉蒂出版于1983年8月1日的小说《Legion》，但当初他并不愿卖出电影拍摄权，直到自己登上了导演的交椅。虽然片名定为“驱魔人3”，但小说和剧本中没有任何驱魔情节，制片方不得不在拍摄中刻意加入了驱魔场景。
